Kate is our South Dartmoor & Totnes First Contact Practitioner Physiotherapist which means that she can see you, the patient, before you see a doctor for any musculoskeletal ache and pains. She can talk about the problem, assess and aim to diagnose then discuss further management and care.
Kate has been qualified since 2007 and has worked in both large hospitals and within the community across all physiotherapy disciplines. Most recently she worked at Derriford Hospital within the emergency and short-term wards and in the Minors department of Accident and Emergency. This experience has allowed Kate to think on her feet and accept that pain is both specific and complex.
Danny is one of our two South Dartmoor & Totnes PCN Paramedics. Danny provides duty list assistance for patients presenting with urgent on the day problems.
With the support of the duty teams they are assessing patients, treating and referring to ongoing services. They are also working towards their ACP Advanced Care Practitioner MSc qualification, which will enable them to add that extra skill set to the duty team and the patients they serve.
Tom is one of our two South Dartmoor & Totnes PCN Paramedics.
Tom is studying for his master’s in advanced clinical practice (ACP). Our Paramedics are very experienced in dealing with acutely unwell patients and making decisions about their care.
He undertakes same day/urgent clinics and home visits for patients that need to be seen that day as their illness or injury can’t wait for a routine appointment. He always has the support of a Doctor and will be able to see and treat an individual or refer onwards to Hospital or another healthcare service as necessary.
Claire is our South Dartmoor & Totnes PCN pharmacy technician. She trained 20 years ago when she lived in Ireland where she worked in community pharmacy. Since returning to the UK Claire worked in prison pharmacy until joining the South Dartmoor & Totnes PCN.
Claire works across the PCN to promote and support medicines optimisation ensuring the effective and efficient use of medicines, minimising clinical risk and reducing wasted medicines, and maximising safe and cost-effective evidence-based prescribing. In addition to supporting the wider medicines optimisation programme, Claire works alongside Annmarie and Jeanne to provide support for medication reviews.
